NCCHC’s clinical guidelines are invaluable for
correctional health professionals who assess and manage chronic
conditions. The addition of three forms for use in daily practice gives
clinicians the tools they need to implement a complete monitoring
program.
The first five clinical guidelines were released in
2001. Since then, eight new guidelines have been developed, including
six for juveniles. They address the chronic conditions most common among
incarcerated patients and those that present the greatest risks if not
well managed. These evidence-based guidelines are reviewed annually and
revised as necessary to reflect the latest scientific findings and
community standards.
The three new forms simplify and standardize
collection of the following:

Baseline medical data for new chronic care
patients
-
Chronic disease clinic follow-up data
-
Clinical data obtained during nursing visits
between clinic visits
The guidelines and forms, along with two companion
documents—definitions of disease control and clinical status and
instructions for using the forms—are available for free at the NCCHC Web
